Our Purpose
The D.R.E.A.M. Community Support Team exists to provide practical, inclusive, and accessible support to individuals living with disabilities and those experiencing barriers within our communities.
Our mission is to promote dignity, independence, hope, and opportunity by connecting people with the resources, services, and compassionate support they need to thrive.
Through collaboration, advocacy, and community partnerships, Dreamers Walk Canada Inc. strives to ensure that no one has to face life's challenges alone.
Core Service Areas
1. Community Outreach Services
Goal
To meet people where they are, build meaningful relationships, and connect individuals with community resources and supports.
Services Include
-
Street outreach and wellness checks
-
Community engagement at local events
-
Connections to shelters, food programs, and support services
-
One-on-one peer support and referrals
-
Crisis awareness and compassionate response
-
Distribution of essential supplies when available
2. Housing, Accessibility & Advocacy Support
Goal
To assist individuals in finding safe, accessible housing while advocating for equal access and inclusion.
Services Include
-
Assistance with housing applications
-
Advocacy for accessible housing and accommodations
-
Support completing ODSP, CPP Disability, and other disability-related forms
-
Accompaniment to housing, medical, and social service appointments
-
Accessibility education and rights awareness
-
Community advocacy and referrals
3. Equipment Support & Mobility Lending Program
Goal
To ensure individuals have access to essential mobility and daily living equipment.
Services Include
-
Donation and redistribution of mobility equipment
-
Wheelchair, walker, cane, and medical equipment lending program
-
Short-term equipment rentals (where available)
-
Equipment maintenance and inspections
-
Coordination of donated equipment
-
Equipment pickup and delivery (when available)
4. Food Bank & Clothing Bank
Goal
To provide individuals and families with essential food, clothing, and hygiene products in a respectful, welcoming, and accessible environment.
Services Include

Food bank distribution
-
Emergency food hampers
-
Ready-to-eat meals (when available)

Seasonal clothing distribution
-
Winter coats, boots, gloves, and blankets
-
Hygiene kits and personal care items
-
Community donation drives

Volunteer Commitment
Community Support Team members are asked to:

Volunteer approximately 3–5 hours per week (flexible scheduling)

Participate in outreach events as available
-
Attend team meetings and ongoing training
-
Represent Dreamers Walk Canada Inc. professionally
-
Complete all required training before participating in outreach activities
